The past few years have been challenging for many people. For some, it has been especially difficult. Uninsured and ill, many people have had limited resources to obtain necessary health care. SW Community Health Center (SWCHC), a volunteer-based safety-net clinic, has been there to provide acute care, preventive health services and chronic disease management to low income uninsured members of our community. For the clinic's clients, SWCHC is their only source of health care, outside of a hospital emergency room.

During fiscal year 2011-2012, community members (both health professionals and individuals with non-clinical backgrounds) provided 2,342 hours of volunteer service to SWCHC. They and SWCHC's small staff worked together to assure quality health services to a vulnerable population.

Since opening on May 3, 2005, SWCHC has provided 12, 276 patient visits to uninsured individuals, over one third of whom are chronically ill (most commonly with diabetes, hypertension or asthma). To meet the needs of the uninsured, SWCHC has expanded access at its existing clinics, introduced diabetes group visits to support its diabetic population as well as offer a monthly Women's Health Clinic and urgent dental clinic. Without SWCHC, patients would not have access to a doctor's visit, necessary medications, prescriptions, imaging or laboratory services.
